HOW CAN I RECEIVE A WIN/LOSS STATEMENT?

You may submit your request using the form on this page. Please include your name, date of birth, address, and Arrow Head Players Club card number.
Your statement will be mailed within about five business days, or you may choose to pick it up at the Arrow Head Players Club.
